A Certified Dementia Practitioner and Aging Well Specialist
We work with clients and families from diagnosis through end of life. We provide expertise and knowledge of Dementia and other Geriatric related illnesses and work together with the client and families using a person-centered approach, it is all about the individual. We help you navigate the uncertainty, and work at guiding and helping to create a clear picture. We start with an overall interview getting to know about the client’s history and family support, We provide a home safety check and assessments – if cognitive impairment is in question, We are certified in administering the MoCA- Montreal Cognitive Assessment, Caregiver Stress Assessment, The Rapid Geriatric Assessment (St. Louis University) as well as a few other screenings if needed depending on the clients needs. We then put together a care plan of action.
The Care Plan will have the client’s scoring and recommendations, helpful techniques/strategies/tools to help with any difficulties or problems and referrals for any outside services. – We grew up in this area and have a group of different resources available to meet their needs – Home Health Agencies, Palliative Care/ Hospice Services, Veteran’s Services, Local Social Service Agency connections, Respite Care, Elder Law Attorneys, Accountants, I know the area Hospitals, Long Term and Rehab facilities, Assisted Livings and Memory Care. If placement is necessary, We can assist with guiding and being as involved in this process as needed. We can be as involved with the client as much as the family needs us. We know that some children of aging parents live in other states or out of the immediate area.
Should a crisis situation arise, and the family needs our assistance and guidance they have all the pertinent contact information to reach us, and we will help in any way.
We also advocate on their behalf with their doctor’s and any other service providers to help them get the best quality of care and quality of life. I monitor the services being provided to make sure they are best meeting the client and family’s needs.
We provide guidance and education to all family members involved via, in person, zoom, if some family members involved are out of state or unable to meet in person. We can connect via Zoom, FaceTime, Google Duo, by phone, email, text.
